    KPMG: Tailwinds and Headwinds for Payers in 2024

    The last year was a big one for payers, with several major M&A deals, according to a recent report from KPMG. This includes CVS Health’s $8 billion purchase of Signify Health and $10.6 billion purchase of Oak Street Health, as well as UnitedHealth Group’s Optum’s $3.3 billion acquisition of Amedisys. In 2024, payers “will become ever more important in the U.S. healthcare system,” KPMG said.

    But there are several challenges ahead as well, such as regulatory pressures against Medicare Advantage and pharmacy benefit managers.

    Here are KPMG’s five tailwinds and five headwinds for payers in 2024:

    Tailwinds

    1. Reduced recession worries: KPMG anticipates slower economic growth in the middle of 2024, but this will improve in the third and fourth quarters with “no recession and no new interest rate hikes by the Fed.”

    2. Premium increases: Premiums are expected to rise “sharply” for some insurance members.

    3. Margin pressures: Both large and small providers are expected to experience margin pressures. This will lead to some being acquired.

    4. Asset integration: There will likely be an increase in “vertical integrations of acquired provider assets,” according to KPMG.

    5. Insurance access: Due to continued high employment rates, commercial enrollment will likely be “maximized,” KPMG said.

    Headwinds

    1. Lending worries: Interest rates will likely stay high, leading to lenders being more wary.

    2. Antitrust scrutiny: Federal antitrust enforcement will continue to be strict. This will end or delay some deals, and raise the costs of some deals.

    3. Talent: “First-rate technical talent” will be challenging to recruit and retain.

    “Companies that innovate to create superior employee value propositions may gain important competitive advantages and create virtuous circles of successful innovation and recruiting,” KPMG stated.

    4. Medicare Advantage woes: Margin pressures are expected to rise for Medicare Advantage plans. This will be due to slower growth and reimbursement challenges from rising scrutiny from the Centers for Medicare and Medicaid Services. In addition, Cigna recently agreed to pay $172 million to settle claims that it sent inaccurate MA diagnostic codes to increase reimbursement.

    KPMG added that in October, experts testified to Congress “that while some MA plans offer important services not available in traditional Medicare, many seniors have trouble navigating more than a hundred options, multiple complications, and misleading and even fraudulent marketing.”

    5. Crackdown on pharmacy benefit managers: KPMG cited a September report from the American Medical Association that found that vertical and horizontal consolidation of PBMs lowers competition. This in turn hurts patients, self-insured employers and insurers not partnered with PBMs.

    “Regulators and legislators may look more closely at PBMs’ role in the drug supply chain,” KPMG said.

After a carb-rich meal, you could gain up to 4 kgs in water weight alone, albeit temporarily. Additionally, a meal high in sodium can lead to water retention, potentially causing a temporary increase in weight overnight. Heavy meals can also cause bloating which too can add up.     2. During Periods:

    During menstrual periods, hormonal fluctuations can cause bloating and water retention. These changes can cause temporary weight gain and might reflect higher numbers on the scale.

    Your weight fluctuates through the day and you tend to weigh your heaviest in the evening. Also, avoid the urge to stand scale every few hours to see if your weight has come down. Your weight can fluctuate up to 2 to 2.5 kgs, and it can happen due to build up of fluids and also depend on what you ate the entire day.     Our body tends to hold more water to repair muscle damage and it might just add up on the scale. Avoiding stepping on the scale a few hours after an intense workout or even the day after.

    6. The day after a festival – Diwali, Christmas, New Year:

    Our festivals all about celebrations and feasting. In fact, indulge yourself during festivals but remember to get back on track right after. Remember consistency is the key!

    Before getting into this review, I wanted to provide a bit of background.

    Paul Anthony and I have a long history; previously, I was a regular contributor to BespokeUnit.com but have been on a hiatus. However, as soon as EDICD launched, I chased Paul down for a box and he obliged.

    It also happens that Paul and I have gone on a number of adventures in the past. The most notable of these involves a bumpy plane ride when I was a baby pilot almost ten years ago.

    Fortunately, we made it through that.

    We both lived to tell the tale and enjoyed a dandy dinner in Miami that night. Since then, I’ve also managed to become a commercial pilot.

    Anyways… back to the cigars.

    So far, I’ve smoked a few vitolas from Every Day Is Cigar Day. They include the Torpedo and the Papito size, which is an even smaller size than the Pablito I’m reviewing here.

    Yet, I’ve become quite fond of the Barberpole Pablito and thought it’d make an excellent subject for this review.

    • Wrapper Hue: Light Brown, Milk Chocolate
    • Rolling Consistency: Slightly Hard
    • Spring: Firm
    • Aromas: Pepper

    So far, I’ve had the chance to smoke three of these Barberpole Pablitos. I did notice minor cosmetic imperfections in some of them, but I personally don’t think they’re too bad. Most importantly, these didn’t affect the smoke at all.

    The Every Day Is Cigar Day cigars are shipped in a cardboard box that is entirely wrapped in cellophane. Inside the box were the 16 individually wrapped cigars as well as a small Boveda 69% pack.

    Upon delivery, I found that the cigars were ready to smoke “right off the truck,” though if you live in more extreme climates, I’d recommend letting your cigars rest for a few days.

    When I received these, it was during a very temperate Oregon summer, so the resting period was not really needed.

    Pre-Lighting Experience

    • Draw: A Little Tight
    • Aromas: Molasses, Straw

    The cold draw has a really lovely molasses and straw flavor which is definitely characteristic of the Connecticut and Habano wrapper. These vitolas had a slightly tighter draw than the previous EDICD sizes I’ve had but I don’t see the need to use my perfect draw on this.

    1st Third Smoking Experience

    • Notes: Black & Red Pepper, Pasta Water

    Upon light up, the predominant flavor is pepper which is very characteristic of Nicaraguan cigars.

    The first third of this cigar surprised me, it’s stronger than it looks and has a really full flavor. It sounds crazy but this cigar on the intake has a really lovely flavor of pasta water with crushed red pepper notes on the retrohale… maybe I’m hungry?

    Transitioning into the second third, more of the Connecticut notes start appearing with notes of honey and whole wheat bread.

    I think one of the hallmarks of a good Barberpole cigar is the dramatic changes of flavor throughout the cigar, and I think this Every Day Is Cigar Day Pablito does a great job.

    Final Third Smoking Experience

    • Notes: Bread, Oats, Coffee

    The last third starts to mellow out, though I still getting notes of honey. In addition to this, a nice creaminess is entering my palate, accompanied by a retrohale of molasses.

    Overall Burn

    • Ash Backbone: Fluffy
    • Burn Angle: Straight & Thin
    • Temperature: Cool
    • Draw: A Bit Tight
    • Final Smoking Time: 60 Minutes

    The overall burn quality of this cigar is very solid. The ash is fluffy, with a flaky and bright white appearance, and the burn line is impressively thin – clear indicators of top-notch tobacco craftsmanship. This vitola also delivers a good volume of smoke, though it feels a bit coarse in the mouth.

    As someone who usually takes their time smoking, I find this cigar perfect for my pace. It consistently lasts me between 45 minutes to an hour, making for a leisurely and enjoyable smoking experience.

    Overall Experience

    Every Day Is Cigar Day’s cigars are sold in a black cardboard box that is quite unique. Most notably, the exterior possesses textured “veins” simulating a tobacco leaf.

    As I mentioned earlier, the entire box is wrapped in cellophane. Additionally, each individual cigar inside also comes in a cellophane sleeve, which you can see through the transparent panel in the front of the box.

    On the other hand, the cigars themselves don’t have a cigar label, which makes them seem a bit plain once unraveled. Yet, if you asked me to pick between a phenomenal smoke without a band and a bland cigar with a stunning band, I’d pick the former any day. When it comes to the EDICD Barberpole cigar, I think it confidently sits in the first category.

    Pairing Recommendations With An EDICD Barberpole Pablito Cigar

    With this Every Day Is Cigar Day Barberpole, I get laid-back vibes when it comes to pairings. For instance, it’d be perfect with some backyard-grilled sausage or Al Pastor tacos.

    Pairing Pablito Cigar With Latte

    As for drinks, a robust German beer or a zesty margarita complements it well. If you happen to be located in Portland like me, I recommend trying it with a trademark oat milk latte; this made for a surprisingly suitable companion.

    Closing Thoughts

    The range of EDICD cigars I’ve smoked so far has consistently impressed me with their outstanding value-for-money proposition and flavor.

    Personally, I’m eager to see the brand explore new horizons with their blends. A Sumatra line would be intriguing, but what I’d really love to see is a Corojo line, as Corojo wrappers are my absolute favorite.

    Have you ever wondered why some people seem to be just so confident?

    You may have asked yourself – “how do they believe in themselves so completely when I am always doubting myself and wondering if I am good enough?” …

    You may have even convinced yourself that they are just more skilled, more talented, more intelligent, more attractive or more experienced than you are and that this is why they can be so confident…

    HOWEVER, often the reason that someone else is more confident than you, isn’t to do with what they know, how smart they are or what they look like. As humans, we all have strengths and we all have weaknesses.  Each and every one of us has things that we do well and are good at and things that we aren’t so great at (or that we need to work on).  When someone isn’t very confident, they tend to spend a lot of their time focusing on those things that they aren’t so great at and beating themselves up for not being better at those things. 

    However, when someone is confident, they tend to celebrate their strengths and positive qualities and then see their weaknesses as areas for development or things that they can work on. Confident people don’t tend to beat themselves up over what they can’t do or because they aren’t good enough, instead they tend to talk to themselves kindly and compassionately, encouraging themselves to succeed in a motivational way rather than a harsh and critical way. 

    Now let me be clear that when I am talking about confidence, I am not talking about being boastful and feeling the need to tell everyone how amazing you are.  I am talking about that quiet internal confidence that means that you aren’t constantly doubting yourself and your abilities. A lot of people can feel as though they aren’t “good enough” or they aren’t “doing enough” and it isn’t their fault that they feel this way. 

    Most people naturally tend to talk to themselves harshly and critically.  Also, how you feel about yourself can often be something that develops from your younger years.  Maybe you had teachers or family members that made you feel as though you weren’t good enough growing up or you have had some other negative experiences that have led you to doubt our worth.  So, it isn’t your fault if you tend to doubt yourself and feel as though you aren’t “good enough”. In fact, many people feel this way.  However, there are things you can do to shift this and to feel better about yourself.

    This is where self-compassion is such a powerful tool for boosting your confidence and self-esteem. Many people tend to be kind and compassionate to those around them but then very harsh and critical of themselves.  When they start to show themselves the same kindness and compassion that they show others, they tend to feel much better about themselves and their confidence improves significantly.

    A first step in nurturing that self-compassion is to start to notice your thoughts and how you are talking to yourself (in your own head) throughout the day.  Are you thinking thoughts and using words to talk to yourself that you would feel comfortable saying out aloud to someone that you really love and care about? Would you tell your child or your sister that “you are such a failure, you are not good enough”? Or would you perhaps coach them by saying something like “you didn’t succeed this time but you can do this, I believe in you, let’s try again and do better next time”.  Aiming to talk to yourself how you would talk to a loved one can be very helpful. Catching and then gently shifting any negative self-talk can really help to boost your confidence and how you feel about yourself.

    There are several other things that can negatively affect your confidence and in these areas self-compassion can help too.  These include:

    Basing your self-esteem on the opinions of others

    So many people rely on other people for validation and confidence.  For example, they will only feel attractive on days when other people have complimented them on their appearance or they will only feel that they have put together a good piece of work when someone else praises that piece of work.  They are relying mostly on the judgment and opinions of others in order to feel good about themselves and their lives.  Basing your self-worth on the opinions of others however makes that self-worth very fragile.  It means that you could feel bad about yourself one day just because everyone around you is in a bad mood and doesn’t feel like complimenting or praising you that day.  It means that you are leaving how you feel about yourself entirely in the hands of other people. Much more helpful than depending on others for validation is to give yourself that validation through treating yourself with compassion. 

    If you are being kind to yourself and celebrating, valuing and appreciating the work you have put in to a project, you won’t need to depend upon other people to tell you that you have done a good job.  If you can appreciate and celebrate your appearance and what you look like, then you won’t have to wait for other people to compliment you to feel good about yourself.

    Spending a lot of time comparing yourself to others

    Often people feel bad about themselves because they spend a lot of time comparing themselves to other people.  Of course, this is never really a fair comparison because they compare the “best” version of someone else (the version that this other person wants to show the world) with the “worst” version of themselves (because only they will know about all of their flaws and insecurities). When making these comparisons people also tend to only make upwards social comparisons – so they will only compare themselves against people that they feel are “better” than them but not against people that aren’t doing as well as them.  This also makes them feel much worse about themselves.  A lot of the narrative that comes up when someone is comparing themselves to others is a very harsh and critical narrative.  They are beating themselves up and being mean to themselves for not being as “good” as someone else. 

    However, nurturing a kinder and more compassionate internal voice can shift that narrative. When someone is being kind to themselves perhaps others become more of a source of inspiration and positive encouragement rather than a reason to beat themselves up and feel bad. Or even better, when someone is being kind to themselves, what others are doing becomes far less relevant or important, because they are just focused on doing the best that they can.   

    Feeling the need to change to fit in

    We are social creatures and we like to know that we fit into our tribe. However, always feeling the need to change ourselves and be something or someone that we are not in order to fit in, can have a very negative effect on our confidence levels and mental health. If we always feel that we can’t be accepted and loved for being our authentic selves we will never believe that we are “good enough” as we are.  Having the confidence to just be you often comes with talking to yourself kindly and compassionately.  Instead of beating yourself up for not being more like someone else, instead you are celebrating and enjoying what it is that makes you unique and different.

    So, self-compassion really lies at the heart of true confidence.  That unwavering belief in yourself and that quiet understanding that you are more than “good enough” comes from being able to celebrate and appreciate your strengths and positive qualities, rather than constantly beating yourself up for your flaws.  How are you going to celebrate what makes you uniquely you today?

    Equipment for influential snaps

    Let’s start with equipment. You of course need a good camera. The options include:

    1. Smartphone camera. A new or new-ish smartphone will be equipped with a high-quality camera and smart photography features. iPhones and Samsung Galaxy smartphones are both great options, though the Samsung may have the edge. Google Pixel is a good budget choice if you prefer to save some money.
    2. DSLR camera: These have a fixed, digital sensor. They excel at autofocus and photos in low-light environments.  
    3. Mirrorless camera: Mirrorless cameras are lighter than DSLRs, because they don’t rely on a mirror to reflect light onto the viewfinder. They have electronic viewfinders that show a digital image of the frame that updates as you adjust settings. Mirrorless cameras deliver excellent image quality.

    Obviously, a good smartphone camera will be your cheapest option. Unless you’re taking photos professionally (or plan to), it usually makes sense to spend your money on accessories to support your smartphone camera. A tripod and external lighting are at the top of that list.

    Editing photos

    Editing social media photos can be a controversial topic. Here’s my philosophy. I’m all for the quick retouch that improves the quality of your images. I feel less supportive of a major overhaul that makes you look like a different person or substantially alters the colors of your outfit.

    But you know? Do what feels right for you and authentic for your feed.

    The basic editing steps to follow include:

    • Cropping: Is your subject framed appropriately and the obvious focal point?
    • Color balance: Are the colors popping? If no, try adjusting the contrast and color brightness. You could also play around with the saturation levels for vibrant or muted tones. Pro tip: Write down these adjustments and repeat them on future photos. This contributes to a consistent and cohesive feed.
    • Photo elements: It happens to the best of us. You think you’ve snapped the perfect image and then, upon review, you see something weird in the background. And that weird thing unintentionally becomes the photos focal point. Fortunately, you don’t have to throw that image away. You can remove unwanted objects online using free tools, which may spare you from reshoots. I used one of these tools recently to get rid of an ugly industrial building in the background of a photo. It was a lifesaver.

    Using natural light

    In my own photography experiments, my biggest challenge is always lighting. Here are some tips I’ve learned that can help you use natural lighting to your advantage:

    • Golden hours: Shoot during sunrise or sunset for warm, soft lighting.
    • Overcast days: Gray skies provide even and diffused lighting. This minimizes hard shadows.
    • Direct sunlight: Head outside on a sunny day when you want high-contrast images with well-defined shadows.
    • Window light: Open the curtains to take indoor shots with a natural touch.
    • Change the lighting source: If the image isn’t coming out the way you’d like, change the direction of the light source. Front-lighting brightens up the subject and back-lighting creates silhouettes and highlights contours. Try tinkering with your exposure settings to amplify these effects.

    Choosing a consistent theme for your feed

    As I alluded to above, a consistent theme separates good Instagram feeds from great ones. You can apply consistent across these areas:

    • Composition: Keep your framing, angles, and subject placements consistent.
    • Color coordination: A recurring color palette across all your posts makes for a lovely profile page.
    • Content type: Post similar kinds of images, such as portraits, landscapes, or food shots. If your Instagram photos vary dramatically in content, your feed can end up looking messy and cluttered.
    • Editing process: As noted, it’s important to follow a consistent editing process. Experiment locally until you find the post-processing style that feels right, then follow the same steps for the pics you upload.

    Those steps support visual harmony across your feed. When that visual harmony also reflects your unique style, followers will reward you. People love consistency in photos as much as they love people to be consistent.

    Your amazing Instagram feed

    It may take some trial-and-error to get your Instagram photo-taking and editing process down pat. (Trust me, I have made lots of IG mistakes, including uploading so-so photos and then later deleting them because they stood out terribly in my feed.)

    Know that your best followers will tell you when you’ve hit the right note. Pay attention to engagement and listen to your followers’ reactions. When one image attracts a bunch of attention, review it carefully to understand why it was successful. If possible, incorporate that success factor into future photos.

    Stick with that process and your Instagram feed will be as amazing as your wardrobe.

    7 Elements to Think About Adding to Your Wedding

    Whether you’re into traditional wedding ceremonies and receptions, or you prefer taking a more casual approach, here are some fun things to consider making a part of your wedding festivities.

    1. A wedding highlight video

    Most people create a wedding highlight video to preserve the most amazing moments from their big day, starting from the time they wake up in the morning. Ideally, you’ll want to hire a professional videographer for the job to ensure you get a high-quality result.

    You can include anything you want in your highlight video, but typically, you’ll want to include all the most important moments, like your vows, first dance, cake cutting, toast, and bouquet toss. In addition to that, many people include the bride, groom, bridesmaids, and groomsmen getting ready.

    You can also get creative and include some funny moments that happen throughout the day. Your videographer will also include some nice B-roll footage of your guests and the general area where you’re having your wedding.

    What you choose to include in your wedding highlight video is up to you, but don’t skip this important way to preserve your best memories from your amazing day.

    2. Break dress code norms

    If you’re into the casual wedding approach, why not also have a casual dress code? Tell your guests they can show up however they feel most comfortable, or have a casual, yet specific dress code where you allow people to wear jeans, but require a nice top. If it’s summertime, swap out the jeans for shorts.

    Some people even extend the casual dress code to their own wedding attire, where brides and grooms wear t-shirts, jeans, shorts, Hawaiian shirts, and sometimes even costumes. One couple threw a costume party and told everyone it was their engagement party, and then surprised everyone by announcing that it was actually the wedding.

    3. Have a themed wedding reception

    If there’s a theme you and your partner really love, why not incorporate it into your wedding reception? For example, say you have a favorite T.V. series you watch together or you share an interest in a particular fandom. Have a cake custom made to represent your shared interest, and plan your decorations accordingly. It’s actually common for people to have Game of Thrones themed weddings, for example, and you can create any theme you like.

    Just be aware that if your theme is too dramatic, some people might not want to attend. Although, that’s really not something you should be concerned about because your wedding is your special day and you should enjoy it to the fullest.

    4. Take song requests

    Sometimes it’s fun to get your guests involved in picking songs for your wedding. When you send out your invitations, ask everyone to write down their favorite song. Give your DJ the list of songs ahead of time and have them make a playlist. Your guests will love it!

    5. Use customizations

    Customizing things is fun, and you can go all out for your wedding. For instance, getting custom labels for your wine will give your guests a really cool experience when it’s served.

    You can also customize anything you want your guests to take home with them, including etched cups, goblets, or even mini champagne bottles. Some people even go as far as customizing cocktail napkins with “fun facts” about the couple getting married.

    6. Get a local bakery to cater your desserts

    It’s hard to find someone who doesn’t love a homemade apple, pecan, or pumpkin pie. There is just no comparison for a freshly baked pie, and if you want your wedding guests to be impressed with your dessert selection, get the best pies you can find.

    Don’t stop with the pies. Look for a bakery that can also make smaller pastries, like almond cakes filled with fruit, butter croissants, almond croissants, and chocolate chip cookies. The more high-end the bakery, the better.

    7. Put your dog or cat in the wedding

    If you have a fur baby, don’t leave them at home; put them in your wedding! Some people task their dog with being the ring bearer, and others just have their dogs and cats walk down the aisle accompanied by a human companion. Including your pet in your wedding is sure to make everyone smile.

    Make your wedding amazing – by your standards

    There is no right or wrong way to host your wedding. Whether you prefer casual or traditional attire, costumes or themes, all that matters is you create your special day the way you will enjoy it best.

    Eyeshadow is my absolute favorite beauty product, so naturally I own 84 eye brushes (actually… I counted).

    I used to throw them all in a cup or mug, but it ended up taking so much time to find the exact type of brush I was looking for. That’s when I embarked on a mission to find 1) a makeup brush organizer with dividers to organize my brushes by type, and 2) the right makeup organization system for ease of use.

    My acrylic storage in shining armor was a 6-slot organizer from Muji. It’s no longer available, but you can find a ton of other organizers of similar shape to fit your needs at Muji or on Amazon.

    Here’s my system for organizing my brushes.

    Front Slot 1: I organize my brushes from largest/fluffiest to most detail-oriented. So slot #1 contains all my big, fluffy blending brushes. These are the brushes I use in every look for transition and crease shades. My favorite brush in this slot—I actually have 3 of them—is the Sigma E35 Tapered Blending Brush. 

    Front Slot 2: Next, I have all of my detail blending brushes. These brushes are fluffy, but smaller (fewer bristles) and denser (bristles packed tighter together). They deposit more color than bigger brushes, but don’t diffuse shadows quite as much, by design. These brushes are great for deepening the crease or outer v or for just blending in smaller areas. They’re also great if you have small eyes, and typical blending brushes don’t work well for you.

    My favorite detail blending brushes are the Sigma E36 Blending BrushMorphe M514 Detail Round Blender, and Sigma E27 Detail Blending Brush. 

    Front Slot 3: Slot 3 is home to my packing/shader brushes. These brushes are flat and dense to deposit intense color on the lids. My absolute favorite shader brush is the Sigma E55 Eye Shading Brush, of which I own 2. I also like the Sigma E21 Smudge Brush for detail shading and Sigma E58 Cream Color Brush for cream shadows.

    Front Slot 4: This slot is one of the most exciting ones! Slot 4 is where I keep all of my detailing liner brushes, angle or definer brushes, glitter tools, and brow brushes. When I use a cake liner or when I need to do detail line work, I reach for one of the fine liners from Suva Beauty, my favorite being the Suva Beauty Six Thirty Liner Brush.

    My secret to smooth concealer/primer application for a half cut crease is to use the spade side of the NYX Dual Silicone Applicator. It’s silicone, so it applies liquid products opaquely and the spade shape of the brush mirrors the shape of the inner corner of the eye. I also use this brush and other silicone brushes, like the Sephora Collection Give Me Glitter Applicators, for applying glitter or very glittery shimmer shadows. Silicone is easy to clean; just wipe off any remaining product. And, now, you don’t have to worry about glitter particles getting stuck in brush hairs of your traditional brushes.

    Rear Slot 1: The slots in the back are where I keep bigger brushes I use less frequently. Rear slot #1 contains all my face brushes. From time to time, I wear foundation or highlight, so I keep these brushes around for that. A big fluffy face brush is also useful for brushing off eyeshadow fallout from your face. I also keep my disposable lipstick wands in a double shot glass in this slot.  

    Rear Slot 2: Finally, the last back slot is where I keep concealer/foundation brushes I use to apply eyeshadow primer. My favorites are the Sigma F65 Large Concealer Brush and *Ecotools Class Foundation Brush from their Blooming Beauty Kit.

    While this system might seem very involved, it makes doing my makeup in the morning so much easier and faster when I know where everything is!
